Normally the Newmar Dutch Star 4369 have the HWH leveling systems with the 11.5" diameter round front and the 13.5" diameter rear feet. RV Snappads have the front ones which I will link below, But they unfortunately have not been able to make a pad for the 13.5" rears. They are working on it for the future but said if the 13.5" has a cut out in the foot, they may not ever have a pad that will fit. If this is not what you have then we would need to know the brand of leveling system and the diameters / shapes of the feet.
