How do i make a warranty claim on this item?

I have part number 73163 that was delivered to me in the middle of October 2018. We removed the cover last weekend and it came off the roof of the camper in pieces. It's like the roof material disintegrated upon touch. It's come apart at the stitching on the sidewalls as well. I see there is a "limited" 3 year warranty on this product. We haven't even had it a year. What does that cover? Am I covered by this warranty?
Please advise.

Below is a link to the Carefree of Colorado websites Warranty information and form. The fabric can be covered under the warranty, I have copied and pasted below what it states about the fabric coverage.
