The mounting hardware can be purchased at any hardware store.
The sizes are going to be:
BOLT - 3/8 - 16 X 1; HEX TAP BOLT GR5 ZINC
BOLT - 3/8 - 16 X 5 1/2; HEX CAP SCREW GR5 ZINC MOTORIZED RAW MATERIAL BLOCK
WASHER - 3/8; USS FLAT WASHER ZINC MOTORIZED RAW MATERIAL BLOCK
FASTEN NUT - 0.375 - 16 NYLOCK ZN GR2 ST
